1. The Three-Part Revision Package
A major- or minor-revision submission usually has three indispensable parts:
Briefly thank the editor, state that you completed the revisions, summarize the main categories of change, and identify the attached response letter and revised manuscript. Give an overview rather than itemized responses.
This is the core document. Reproduce every comment from each reviewer and answer them one by one without omissions. Reviewers focus on it during the next round.

2. A Four-Part Response to Every Comment
Use the same four-part structure for each item so reviewers can immediately see how it was handled:
- 1.Quote the reviewer's comment verbatimand number it R1-1, R1-2, and so on, so reviewers do not have to search the previous report.
- 2.Thank the reviewer and state your position. Agree explicitly when you agree. If you disagree, explain politely and frame the issue as insufficient explanation rather than reviewer misunderstanding.
- 3.Describe the exact change. State whether you added a comparison, ran an experiment, or rewrote a passage; do not say only that it was revised.
- 4.Quote and locate the revised text. Include the new sentence and its section and page so the reviewer can verify it.
The editor sees publication potential but substantial problems, often involving methods, experiments, or the central contribution. Responses must be detailed, and substantive literature or method issues must be solved rather than cosmetically rephrased.
Comments usually concern writing, format, or local clarification. Address each one cleanly and concisely; new experiments are less commonly required.
3. Five Comment Types and Their Responses
Different comment types require different actions. Literature and evidentiary-support comments often take the longest because no template can supply the answer—you must read recent work.

4. Answer Literature Comments with Substantive Searches
“Novelty is limited,” “compare with recent work,” “related work is incomplete,” or “why did you not cite X?” are difficult because wording templates cannot answer them. You must find recent studies, understand what they did, and explain precisely how your contribution differs.

Suppose a medical-informatics manuscript proposes a RAG method for Chinese clinical QA with a post-retrieval fact-verification layer to reduce hallucinations and receives this major-revision comment:
Reviewer 2. The manuscript applies retrieval-augmented generation to medical QA to reduce hallucination. This direction has been widely explored and the novelty is limited. The related work omits recent medical RAG systems, and there is no comparison against them. Please clarify the contribution.
- MedRAG / MIRAGE(Xiong et al., ACL 2024 Findings) introduced the 7,663-question MIRAGE benchmark across five medical QA datasets and the MedRAG toolkit. It reported gains of up to 18% over chain-of-thought prompting across six LLMs and identified a lost-in-the-middle effect in medical RAG.

Frequently Asked Questions
How long should a response letter be?
There is no uniform length. Answer every comment independently and identify the corresponding place in the manuscript. A major-revision response may range from a few pages to more than ten.
How quickly should I respond?
Follow the editor's deadline, which is often several weeks to several months for a major revision. If experiments cannot be completed in time, politely request an extension. Most journals accept reasonable requests, and a defensible revision is better than a rushed one.
What if I disagree with the reviewer?
You may disagree, but provide evidence rather than emotion: cite literature, add data, or run experiments, and thank the reviewer for raising the issue. Avoid saying the reviewer misunderstood; say that the manuscript was unclear and now includes an explanation at a specific location.
Does a major revision guarantee acceptance?
No. It means the editor sees publication potential but substantial issues. Solving every issue and reflecting the changes in the manuscript improves the odds, but another round or rejection remains possible.
